Your Full Name Home Address Date of Birth Email Address This scholarship is for $100.00 to the following accredited institutions: (1) Two or four year colleges, (2) trade or technical schools, (3) business colleges,(4) Nursing schools, and (5) specialized training (such as beauty schools.) Qualifications: A minimum GPA of 2.0, all applications will be reviewed by Deb and Bill Jordt, with recommendations from the High School Counselor. This is eligible to one senior boy and one senior girl. Applications must be submitted by May 1. Respond to the following questions with clear and brief answers. 1. Why do you believe you should receive this scholarship? 2. List any hobbies or special interests you have. 3. List any school activities in which you have participated during high school. 4. List any community or church activities in which you have participated during high school. 5. What career goal(s) do you have at this time? 6. Name the location of the school or schools where you plan to continue your education.
